Soulful Needs Assessment Survey
In the HSCV321 course, "Community Health," the class is divided into sub-population groups in which we are to investigate community health issues, resources &/or disparities related to our selected sub-population.
We already recognize that health resource sensitivity to the small population of African-Americans in Butte County are limited. We are planning to address this issue and attempt to develop a plan to enlighten & encourage the community to revise their practices and consider bringing a health resource center specific to the health needs common to our sub-population.
In order to devise a specific plan, we need to implement a Needs Assessment. A needs assessment focusing on the African-American (or Black) population in Butte County has been created on SurveyMonkey and we need data.
The collected data is strictly anonymous and will not be used for advertising nor selling purposes.

If you can take two minutes of your time to answer 10 simple questions, we can begin to develop a plan to address African-American health Care disparities in Butte County.
We are hoping to gain enough responses to close the survey next Thursday.
