Make an Appointment
You may either call us at 530-898-6345 or stop by our office which is located in the Student Services Center 430. The receptionist will ask you for your name, student ID, and a phone contact when you make an appointment.
At the time of your first scheduled appointment (which usually lasts 50 minutes), you will need to arrive 20 minutes early to complete some paperwork which includes:
- An informed consent form which discusses your rights to services, what you may expect from counseling, confidentiality policies, and a brief description of the counselors
- A client information form that asks for demographic information, personal health/counseling history, and reason(s) for coming to counseling.
- A brief assessment tool that considers your symptoms during the past two weeks.
Most students schedule regular appointments which may be within one to two weeks away depending on what time of the semester it is. However, if you have an urgent concern and need to see a counselor today, you may come in for an assessment during crisis hours, Monday through Friday, 9:00 a.m. to 4 p.m.
