During your fall semester, you should complete and submit
your application for Certificate of Clearance. The packet is available in the
Department of Education office, Tehama 101. Carefully follow the instructions
on the cover page.
At the end of your fall semester, you should visit the
Multiple Subject Program website at: http://www.csuchico.edu/educ/progs_multiple.html
Download the Department of Education application. The application for admission
to the credential program is a one-page form plus the Professional Goals and
Autobiographical Statement sheets and Disposition form. Download those forms
from the link at the bottom of the page.
The following are instructions for students in the Integrated
Teacher CORE (other lists and materials on the Department of Education site may
or may not be applicable):
During January, complete the credential program
application packet and turn it in to me at my office (Tehama 104) on or before
February 1. The contents for ITC students should be in this order:

check for $50 made payable to ÒCSU, Chico.Ó This is your credential program
application fee.

copies of your resume

Belief Statement

Experience Statement
6 Two
letters of recommendation that attest to the attributes you possess that will contribute to
effective teaching. These can
be from a former teacher, from a work supervisor or person in the community, for example.

references, with name and contact information, of people who know you in an educational setting.

Candidate Disposition Form should be given to the teacher in whose classroom
your early school experience was done or another teacher who has seen you work
with children. The teacher should complete the form and mail it to the
Department of Education. You must fill in the top portion before giving
it to the teacher.
You
should receive a preliminary letter some time in March and the final word on
your acceptance by the beginning of April. Carefully follow any directions on
these letters and make sure that you attend the information meeting at the end
of the semester.
If you have not yet taken
CBEST and CSET, you should do so as soon as possible. You must
completely pass CBEST before starting Practicum I. You must completely pass
CSET before starting Practicum II.
