Forms and Applications

Most forms and applications are provided in Adobe Acrobat PDF format. Download the free Acrobat Reader if you don't already have it. Please see below to locate the form you need. Please make sure you choose the form for the correct year.

Instructions for PDF forms

  1. IMPORTANT:  Before submitting all documents, print your name and Chico I.D. on the top of each requested item.
  2. All documents received will be scanned and linked to the student's file.
  3. Paper documents are retained for one year and then shredded.
  4. Please submit only copies and keep originals for your records.
  5. Do not send documents by e-mail, as e-mail is not a secure means of communication.
  6. Items submitted by fax should automatically update your Student Center "To Do List" from "requested" to "received" within 72 hours.  If you have any questions after 72 hours, please call us.  Please do not resend additional copies unless specifically asked to do so.
  7. Items submitted by mail or delivered to our office should update your Student Center "To Do List" from "requested" to "received" within five weeks.  If you have any questions after five weeks, please contact our office.
  8. You must submit all requested items on your "To Do List" by June 1st in order to have your file complete before the first fall disbursement.
  9. Check your Student Center on a weekly basis after your forms show as received.  If we need any additional information, it will show up on your "To Do List." Once the requested items have been reviewed and completed, they will be removed from your "To Do List."

Instructions for Electronic forms

  1. Several forms are presented in electronic format.
  2. For the electronic forms, when you click on the form link, you will be directed to the Chico Portal Login.
  3. Your portal Username and Password will serve to authenticate you in our system.  If you do not have a FAFSA on file for the aid year you select, you will receive an error message directing you to submit a FAFSA.
  4. Portal authentication serves as your signature.  You will receive a Confirmation page for the electronic form you submit as well as an e-mail confirming submittal.  It is not necessary to print these pages, but you may do so if you prefer.
