1. IMPORTANT:  Before submitting all documents, print your name and Chico I.D. on the top of each requested item.
2. All documents received will be scanned and linked to the student's file.
3. Paper documents are retained for one year and then shredded.
4. Please submit only copies and keep originals for your records.
5. If you choose to submit documents by fax, please call our office 10 minutes afterward at 530-898-6451 to confirm receipt and legibility of the fax transmission. Due to the large volume of documents we receive, we will not be able to provide this information if you call at a later time. Please do not resend additional copies.
6. Do not send documents by e-mail, as e-mail is not a secure means of communication.
