FMS Service Guide

    This guide is provided in order to assist you in understanding what services Facilities Management & Services can provide and how to obtain such services. FMS does not provide maintenance to the Residence Halls, Housing, or Farm buildings.

    SERVICE CATEGORIES
    The University community may obtain services from Facilities Management & Services by phoning ext. 6222 or by submitting the appropriate request form.  Services are classified into the following categories:
    Service Requests: Maintenance services provided free
    Project Requests: Non-maintenance services provided at cost
    Special Services

    SERVICE REQUESTS
    Service requests are for maintenance services as defined by the CSU system.  Maintenance services are defined as basic, recurring work necessary to maintain the University facilities in good repair and in satisfactory operation.  These services include maintaining, operating and repairing utility and building systems such as electrical systems, water, gas, heat, ventilation, plumbing, sewage collection, and elevators plus any equipment attached to these systems. It also includes maintaining and repairing the basic components of the University's  buildings and environs such as foundations, walls, roofs, stairs, ceilings, floors, doors, windows, hardware, turf, sidewalks, streets, and ancillary facilities or equipment.  The CSU system classifies the equipment that Facilities Management & Services maintains as Group I Equipment.  An individual requesting services is asked to provide the following information for any service request:
         1)   description of service request
         2)   location of problem
         3)   the requester's name, telephone number and department

    TYPES OF SERVICE REQUESTS
    PHONE REQUESTS
    Any disaster, unusual occurrence, utility malfunction or equipment failure which presents an imminent danger to life, health or property is an emergency and should be reported immediately to Facilities Management & Services at ext. 6222, or after hours to the University Police Department at ext. 5555. Maintenance services for simple failures of broken or malfunctioning equipment for which Facilities Management & Services is responsible but pose no imminent danger should be reported by phone. Services that need immediate attention, for example broken windows or rooms too warm or cold, should also be reported by phoning FMS at ext. 6222.

    WEB REQUESTS
    Maintenance services which pose no imminent danger and do not need immediate attention may be obtained by submitting an On-line Service Request. Moving services and requests for relocating staff, offices or classrooms should be submitted using an On-line Service Request five to ten business days prior to the move.

    Priority of Work- Maintenance work takes priority over non- maintenance work. Maintenance work is prioritized as follows:
    Priority 1. Emergency Repairs
    Priority 2. Other Repairs
    Priority 3. Maintenance Services and Moving Services

    PROJECT REQUESTS
    Project requests are for work considered non-maintenance and funded by the requesting college or department. Facilities Management & Services will either perform the non-maintenance services requested or serve as the requesting department's agent in obtaining the services from a private vendor. Examples of non-maintenance services include but are not limited to the following: a) Installation and repairs to department equipment (Group II Equipment); b) Fabrication, installation and repair of teaching aids; c) Furniture fabrication; d) Picture framing; e) Painting when done at a time other than planned for in the regular painting cycle; f) Remodeling of assigned areas as requested by colleges, schools, departments or agencies; and g) All services provided to auxiliary and non-state funded agencies/organizations.     Project Request Form- The Project Request Form is used by a department to initiate a project request. Copies of the Project Request Form are available on the web or can be obtained from Facilities Management & Services. The form must be signed by the requester, the requester's Dean or manager and the appropriate Vice President prior to being transmitted to Facilities Management & Services. The requests are processed  in the order recommended by the Vice Presidential Designees who assign priorities to best meet the mission and strategic plan of the University.

    Project Estimate & Work Authorization- Cost estimates are provided to the department for projects prior to work beginning. An estimate is provided on a FAST TRACK form or a Project Estimate & Work Authorization Form (PEWAF). For projects estimated under $5,000 the department sends a CAF (Charge Authorization Form). For Projects over $5,000, funds are encumbered. Work will not commence until Facilities Management & Services is in receipt of a fully executed PEWAF.

    Bicycles in Buildings- No one is permitted to walk or park a bicycle, unicycle or motorized bicycle in any University building unless such area is specifically designed for bicycle parking. Bicycles found illegally parked in any University building will be impounded by the University's Police Department.

    Bulletin Boards- Bulletin boards must comply with University standards prior to installation by Facilities Management. Bulletin boards are difficult as well as costly to relocate. If at all possible, it is appropriate to install a new bulletin board at a new location rather than moving an old one to the new location. A copy of the University standards for bulletin boards is available from Facilities Management & Services. Bulletin boards to be installed in building corridors or hallways must be of fire-resistant construction per California Fire Code regulations. Due to this regulation, the mounting of holders for handout materials and pamphlets on bulletin boards is discouraged. Bulletin boards may be obtained from Facilities Management by using the Direct Billing process or by submitting a Project Request Form.

    Clocks- The University operates a master clock system. Any malfunction of the system or an individual clock connected to a master clock should be immediately reported to Facilities Management & Services at ext. 6222. Facilities Management & Services is not responsible for the maintenance of non- system clocks.

    Electrical Connections- Please do not try to repair electrical equipment yourself. Instead, contact Facilities Management & Services at ext. 6222 for help. Facilities Management & Services should be contacted prior to any selection of equipment which will be connected to the University's electrical system and used in a University facility. All equipment is required to comply with University electrical requirements. Facilities Management & Services is also required to record the use of such equipment for University energy consumption reports.

    Elevators- Trouble calls for elevators out of order should be made to Facilities Management & Services at ext. 6222, or after business hours to the University Police Department, ext. 5555. FMS administers the service contracts with private vendors for the maintenance of the University's elevators. Therefore, any complaints concerning services provided by such private vendors should be provided in writing to Facilities Management & Services. Unauthorized personnel should never attempt to adjust or modify the operation of the elevators.

    Events or Reservations of Facilities- All event requests, facility reservations, and equipment needs are to be made using a Facility and Equipment Reservation Form which is submitted to the Facility Reservations Office at zip 116, ext. 6118. If the request involves using University equipment or FMS services, please submit a request to Facilities Reservation ten days prior to the event.

    Fire Extinguishers- Fire extinguishers are maintained by Facilities Management & Services and are coded as to their location. Please do not relocate or remove any fire extinguisher except in case of fire emergency. Please immediately report the discharge of any fire extinguisher.

    Key Issue- University keys are issued and maintained by Facilities Management & Services. Faculty and Staff can request keys on a pink Key Authorization Form. Keys requested for students require a yellow Key Authorization Form. These forms must be signed by a Dean or a Department Head prior to issuance of keys. The key authorization forms as well as key return envelopes may be requested by phoning Facilities Management & Services at ext. 6222. Interdepartmental trading of keys is not permitted. To ensure you are not responsible for a key no longer in your possession, it is important that you make sure the keys you are issued are returned to FMS. The University has established a $25 key replacement fee for keys you may lose. Please contact the Key Shop at ext. 6222 if you have questions.     Limitations- Facilities Management & Services personnel, materials and/or equipment are prohibited from being used for private or personal benefit either on or off campus.

    Mechanical/Fan/Electrical Rooms- Access to mechanical, fan or electrical rooms is restricted to Facilities Management & Services personnel. No materials are to be stored in these rooms. Noises or fumes originating from such rooms should be immediately reported to Facilities Management & Services at ext. 6222.

    Occupants' Responsibilities- University occupants are responsible for turning out lights, keeping windows closed, locking doors and securing their rooms and areas. Occupants are requested not to make any permanent modifications to any room or area without prior authorization by Facilities Management & Services. No items should be attached to any walls or doors in any hall or any building without prior authorization by Facilities Management & Services. Occupants are encouraged to utilize bulletin boards for posting of any information. Custodians are not permitted to clean personal rugs placed in any office or room.

    Opening Buildings- Custodians are responsible for unlocking all University buildings and classrooms for normal University schedules. Faculty and departmental offices will only be unlocked by University Police Department personnel. Facilities Management & Services personnel are not permitted to open offices for students.

    Painting- Painting services are performed on a scheduled basis, generally on an eight year cycle. Any request for painting which reduces the time of the cycle must be charged for through the Facilities Management & Services reimbursement system.

    Protection of Chico Creek & Creek Bank- The University is enhanced by the presence of Chico Creek running through its environs. No one is permitted to dump, drain or dispose of any waste, chemicals, solids, litters, or any other material into the creek. Also, no one is permitted to modify any portion of the creek bank in any manner without prior written authorization of Facilities Management & Services. Please note that all of our storm drains run directly to the creek. Rain water is the only item permitted in storm drains.

    Reporting Service Complaints- Our objective is to provide requested services in a timely and satisfactory manner. If you are dissatisfied with our service, please notify the work Control Center at ext. 6222. You will receive a response to your complaint and we will make every effort to provide you the service requested in a satisfactory manner.

    Roofs- In order to prevent damage to the roofs of University buildings, no persons are permitted on building roofs without prior authorization by Facilities Management & Services unless the roof is specially designed for foot traffic (such as the roof of Holt Hall). No one may place any equipment on any building roof on campus without prior authorization by Facilities Management & Services, and then such access should be limited to the specific area designated by the FMS as accessible.

    Signs & Banners- Permanent signs placed either inside or outside of University facilities must be approved by Facilities Management & Services prior to their installation. Standards describing the characteristics of the signs are available through Facilities Management & Services. Temporary signs not posted on bulletin boards or approved by Facilities Management & Services will be removed by University maintenance or custodial personnel. Banners must be approved by the University Campus Planning Committee. Please contact Facilities Management and Services to coordinate approval prior to placement.

    Special Custodial Services- Special custodial services are available by contacting the Facilities Management & Services office. Reports of building problems may be made to custodians who will then report such problems to the Facilities Management & Services office.

    University Grounds Care- The natural environment of the University is maintained by the Facilities Management & Services personnel assigned to Grounds Maintenance Services. Grounds personnel are not permitted to prune or propagate any plants or trees for the personal use of anyone. Notification of grounds or landscaping problems can be made by phoning Facilities Management & Services at ext. 6222 or by submitting an on-line service request.

    Use of University Trees- In order to preserve the natural beauty of our campus, no connections, bolts, nails, or other objects may be attached to any tree on the University's grounds without the prior written authorization by Facilities Management & Services. Bicycles, unicycles, motorcycles or motor scooters are prohibited from being parked against or locked to a tree on the University's grounds.

    Work for Non-State Funded Organizations- Facilities Management & Services is not funded to provide maintenance services (respond to service requests) to University or ancillary organizations which are not funded by the State of California's General Fund. Services provided to non-state organizations and auxiliaries will be performed on a reimbursement basis.
