Note: Please save the PDF file first. Then open the saved file and complete the form using your computer. The preferred method of delivery for Room/Space Use Requests are to email your saved form. Alternatives include emailing a scanned copy or placing a printed completed in campus mail.
Please do not send FRES multiple copies of the same request.
Utilize this form for ANY room request where additional equipment
(including, but not limited to: tables, chairs, etc.) is NOT needed.
IF you need a space or room with additional equipment please use the
Facility and Equipment Request Form (FERF) below.
Use in conjunction with the FERF when reserving a theatre or needing media services/equipment and returned to Facilities Reservations.
Complete when the campus tent is requested.
Complete in conjunction with the FERF when self preparing/cooking food for your event.
Complete when the President's attendance is desired, form is returned to the President's Office (KNDL 105).
Complete for any recycling needs (free services from AS), form is returned to AS Recycling (BMU).
Complete at any event that a volunteer is needed for a campus event, form is returned to Human Resources.
Complete for any event scheduled at the Warrens Center, form is returned to Facilities Reservations along with layout map and CAF for payment. Facilities Management Services (FMS) requires 10 working days notice prior to the event.
