Room/Space Request Form (Pre-Reservation Info Form)

Please use this form for all reservation requests to assist the Facilities Reservations staff in responding to your request in a timely manner.

Facility and Equipment Request Form (FERF)

To be completed for any reservation request needing services/equipment and returned to Facilities Reservations. Facilities Management Services (FMS) requires 10 working days notice prior to the event.

AV Technology/Theatre Use Planning Form

To be used in conjunction with the FERF when reserving a theatre or needing media services/equipment and returned to Facilities Reservations.

Campus Tent Request Form

To be completed when the campus tent is requested.

Food Serving Permit Application

To be completed in conjunction with the FERF when self preparing/cooking food for your event.

President's Attendance Request Form

To be completed when the President's attendance is desired, form is returned to the President's Office (KNDL 105).

Recycling Request for Special Events Form 

To be completed for any recycling needs (free services from AS), form is returned to AS Recycling (BMU).

Volunteer Employee Form  

To be completed at any event that a volunteer is needed for a campus event, form is returned to Human Resources. 

Warrens Center Request Form

To be completed for any event scheduled at the Warrens Center, form is returned to Facilities Reservations along with layout map and CAF for payment.  Facilities Management Services (FMS) requires 10 working days notice prior to the event.  Warrens Center Guidelines
