Quizzes: Twelve quizzes will be administered throughout the semester on assigned textbook readings. All twelve quizzes will count towards your grade. Each quiz is worth 20 point, for a total of 120 points available for this component of your grade.

Note Cards: Read each chapter at the assigned time and submit one note card at the beginning of each class session (5x7 or larger). Note cards will be collected at the beginning of each class period. No Late Cards Will Be  Accepted. The note card assignment has two components. 1) Facts you might want to remember and 2) the most important information you learned in the chapter and why. You will be permitted to use your note cards during the quizzes. Be sure to put your name and chapter title on each note card you hand in. On the front side write down the main points of the chapter, those you want to remember for the quizzes. On the reverse side discuss a particular piece of new information which makes you think about its effect upon your own aging process, or how the information impacts your professional life. You Need To Fill Both Sides of The Note Card For Full Credit (5 points eachtotal=60).

Health Education Fact Sheet: Each student will be required to create an 8 ½ by 11 inch (standard size) fact sheet on the chronic disease or acute illness of their choice for use by older adults in the Butte County Area. The fact sheet health topic must be approved by the instructor and selected by the student by September 24, 2007 or earlier. Presentations by all students of their completed fact sheets will be during final class meeting on December 17, 2007.  When developing the content and design of this fact sheet the student is asked to maintain an awareness of the population who will be accessing the fact sheet for health information i.e.: size of font etc… This assignment is worth 50 points toward your final course grade.

Chronic Disease Paper: Each student will write a 2-4 page paper on the chronic disease their choice which affects elder adults (over 60yrs of age). Please follow APA format, minimum four references required. This assignment is worth 50points.

Syllabus Change:

The instructor reserves the right to make changes as necessary to this syllabus. If changes are made, at least two weeks of advance notification will be given to the class. Changes will be announced and posted on the course portal account.

Academic Honesty:

The guidelines for the CSU Chico Academic Honesty Policy, as stated in the University Catalog will be fully adhered to in this course. Grades and degrees earned by dishonest means devalue those earned by all students; therefore, it is important that students are aware of CSU Chico Academic honesty policy as stated in the University Catalog.

Accommodations:

Students with disabilities requesting reasonable accommodations must first register with Disability Support Services (DSS). DSS provides documentation to students with disabilities who must then provide the documentation to their course instructors. Students with disabilities should notify their course instructors of their need for reasonable accommodation in a timely manner to ensure sufficient time to arrange reasonable accommodation implementation and effectiveness. A typical timeframe for arranging reasonable accommodations for students who are registered with DSS is approximately one to two weeks.

Classroom Behavior:

Appropriate classroom behavior is expected of the instructor and all students. Inappropriate and disruptive classroom behavior including inappropriate language and gestures, class disruption, disrespect to other students, and other behavior as determined by the instructor will not be tolerated and will result in possible removal from the class and/or disciplinary action as per the university catalog.

Attendance:

Every student is expected to attend class, arrive to class on time and participate in-class discussions, proposed questions and activities. You are allowed to miss two three-hour classes without penalty. After missing two classes, each additional class or partial class missed will result in a 5% grade reduction. If you are going to miss a class, you should contact the instructor ahead of time by calling 530-898-5086, and leave a message with name and phone number.
