Major/Minor Clearance Forms

You can download the forms you need to apply for graduation. Each document is in .PDF format. To view these files, you will need Adobe Acrobat Reader. If you don't have a copy of this program, you can download one for free. These forms are interactive and can be completed and edited online. When you are finished, print out three copies of the form you have completed, and bring it in to the History Department.
