Ready Set Go

Open enrollment is your once-a-year opportunity to evaluate and make necessary changes to your benefit programs. It is an opportune time to view health costs and plan changes and decide what best fits your needs. If you do nothing, your medical, dental, and vision enrollment will continue into the 2019 plan year.

What's New for 2019

  • PERS Select has adopted a value-based insurance design.  The plan will change co-pays for primary care, specialists and urgent care; award credits to reduce the annual deductible;  eliminate hospital tiering; and eliminate coinsurance for inpatient delivery after enrollment in the Healthy Moms program.
  • Delta Dental PPO Plan Enhancements:
    • When you visit a PPO dentist, your diagnostic and preventative services (like cleanings and exams) will not count against your annual maximum.  This benefit does not apply if you visit a Delta Dental Premier or non-Delta Dental dentist.
    • SmileWay Wellness Benefit is an enhanced coverage for higher risk employees and their dependents.  Employees and their dependents with certain qualifying medical conditions will be offered expanded coverage.

Required Documents

To request any open enrollment change, please submit the Benefits Worksheet, along with the required supporting documents to the HRSC Benefits Office, Kendall Hall 220.  Benefits Open Enrollment forms can only be accepted between Monday, September 10 through Friday, October 5, 2018 by noon. All changes requested become effective January 1, 2019. If you do nothing, your medical, dental, and vision enrollment will continue into the 2019 plan year.

Reminder

The Dependent Care Reimbursement Accounts and Health Care Reimbursement Accounts require annual re-enrollment. Please complete the 2019 Health and Dependent Care Reimbursement Account Form and submit it to the HRSC Benefits Office, Kendall Hall 220. Benefits Open Enrollment forms can only be accepted between Monday, September 10 through Friday, October 5, 2018 by noon.
