Annual Open Enrollment
The annual CSU open enrollment period (September 15 through October 10, 2014) is an opportune time to think about your benefits and what makes the most sense for you and your family.
This is your once-a-year opportunity to evaluate and make necessary changes to your benefit programs, unless you experience a mid-year Qualifying Life Event.
Detailed information regarding all CSU core and voluntary benefit plans can be found on the CSU Systemwide Benefits Portal. You can also click on portal picture on right to go to the Systemwide Benefits Portal site.
During open enrollment you can enroll, change, or cancel the following benefits:
- CalPERS Health (2015 Plan/Rates)
- Dental (2015 Plan Brochure)
- Vision (Brochure)
- FlexCash Plan and Tax Advantage Premium Plan (TAAP) (Brochure)
To enroll in or make a change to the above benefits, please complete and submit the Benefits Enrollment/Change Worksheet & HBD-12A Form, along with any required supporting documents to the HRSC Benefits Office, Kendall Hall 220, no later than 5 p.m. on Friday, October 10, 2014.
You may also enroll (or re-enroll) in the following Voluntary Benefit Plans:
- Dependent Care Reimbursement Account (DCRA) - 2015 Plan Year [Annual re-enrollment required]
- Health Care Reimbursement Account (HCRA) - 2015 Plan Year [Annual re-enrollment required]
To enroll (or re-enroll) in DCRA and/or HCRA, please complete the DCRA/HCRA Plans Enrollment Authorization form and submit it to the HRSC Benefits Office, Kendall Hall 220, no later than 5 p.m. on Friday, October 10, 2014.
January 1, 2015 is the effective date for enrollment and/or changes made during the open enrollment period. If you do nothing, your medical, dental, and vision enrollment will continue into the 2015 plan year.
