|Re:||State Employee W-2’s for 2013|
Missing W-2's Were Mailed Feb. 12
The good news is that the W-2’s have been printed and mailed by the State Controller’s Office (SCO). Affected employees should receive the W-2’s within the next few days. The W-2’s printed yesterday should have printed during the initial run but did not. Additionally, the SCO assured California State University that there was no breach of personal information.
A printer malfunction was the cause of missing W-2’s
The State Controller’s Office (SCO) confirmed today that approximately 3,900 W-2’s had not printed as a result of a printer malfunction. This incident affected a majority of CSU Chico employees. The SCO confirmed today that the W-2’s have been printed and mailed and that affected employees should receive the W-2’s within the next few days, most certainly by the end of next week.
Action: Request a Duplicate W-2, through HRSC, if not received by Feb.21
We need to know if you do not receive your W-2 by Friday, February 21, 2014. The campus will submit a request to issue a duplicate W-2 to the State Controller’s Office (SCO) on your behalf. If duplicate W-2’s are requested as a result of this incident, the SCO will waive associated fees, including those for individual requests.
Duplicate W-2’s will be issued by the SCO at no cost to the employee provided that the address on file is correct (as of the 12/12/2013 deadline). If the address on file is incorrect because the address change was not reported timely, the employee must contact the SCO directly and a $8.50 fee will charged to the employee for the duplicate W-2, payable to the State Controller’s Office. Additionally, the SCO will not waive fees for employees who reported receiving a damaged W-2 (e.g., as a result of possible mis-handling through the mail.) Please contact the Human Resources Service Center and/or the SCO as appropriate for additional information.
Please go to our W-2 Information web page for more information. We will update our FAQs with a process for notifying us if you have not received your W-2 by February 21, 2014 and provide more information as we receive it.
