Fee Waiver Applications
The Fee Waiver Application period for the Spring 2010 semester is from OCTOBER 5 through NOVEMBER 6, 2009.
You can find information on the Program, the requirements, eligibility guidelines, the application, and other required forms at the following site: http://www.csuchico.edu/payr/feewaiveroverview.php. To ensure quick processing, please obtain all required signatures and complete ALL questions on the form before delivering to the Payroll and Benefits office, Kendall Hall Room 222. Incomplete forms will be returned to the employee and approval may be delayed. If you are eligible for fee waiver and are not using the fee waiver for yourself, you may be able to transfer it to an eligible spouse, domestic partner, or dependent child who is matriculating towards a degree or teaching credential at any of the CSU campuses at greatly reduced rates.
PLEASE NOTE: All Fee Waiver Applications for the Spring 2010 semester must be completed and returned to the Payroll and Benefits office, Kendall Hall Room 222 NO LATER THAN FRIDAY, NOVEMBER 6, 2009. PPO HEALTH CARE PREMIUMS FOR SEPTEMBER AND OCTOBER

This only applies to CSU, Chico State Employees currently enrolled in PPO policies, not HMO plans.
CalPERS has granted two health care Holiday Premiums. The premium holiday is scheduled for the months of September and October. For employees enrolled in these three plans (PERSCare, PERS Choice, and PERS Select) both the employee and employer deductions will be reduced to zero on these two monthly pay warrants. The campus has established a Web site for furlough information -- http://www.csuchico.edu/hr/furlough -- that will be a location for calendars, FAQ sheets, documents, forms and other materials related to furloughs affecting CSU, Chico and the CSU. The site is linked from the University home page – www.csuchico.edu – next to the link for the campus budget information Web site -- http://www.csuchico.edu/prs/budget/index.shtml. Some of the information on the site will pertain to staff and other information to faculty – please read the titles and links carefully to see if schedules, forms or other documents pertain to you. An e-mail address – furlough@csuchico.edu -- has been set up for questions that you may have. One of us from our two offices will respond to you as quickly as possible.
We recognize that the scheduling of and planning for furloughs is complicated, and made all the more so by our semester starting in a few days. Please know that we are working as quickly and carefully as possible to communicate furlough schedules and procedures to you. Some of you have information already from your divisions or bargaining units regarding furloughs. Please check to make sure that information is accurate by referring to the furlough Web site linked above. Because of the unprecedented nature of implementing furloughs at CSU campuses, some plans have had to change almost daily as we consult with the Chancellor’s Office.
Please note: Furlough days for August have already occurred on August 7 and 14. Looking ahead to September, the campus will be closed for a furlough day for faculty and staff on Tuesday, September 8. Staff and 12-month faculty must also take a floating furlough day in September using processes addressed in documents available at http://www.csuchico.edu/hr/furlough.

CHANGE IN CAMPUS WORK SCHEDULE

Earlier this year the campus adopted a 4/10 work schedule for the summer with the anticipated return to a 5/8 work week scheduled to occur August 14, 2009. Due to the decision by the Trustees to adopt a furlough program for MPP, Confidential and Executive employees and to the ratification by CSUEU members of a Memoranda of Agreement to adopt furlough, the following change will be made.
Employees whose bargaining representative has agreed to a furlough program as well as confidential employees, MPP employees and executive employees will adjust to a 4/8 hour work schedule for the period July 31, 2009 - August 14, 2009. (See work schedule calendar at http://www.csuchico.edu/hr/Documents/SHRM-AugustWorkCalendar.doc .)
All other employees will remain on a 4/10 hour schedule until August 14th as originally planned. (See non-furloughed work schedule calendar at the link listed above.)
The campus will return to the 5/8 hour schedule after August 14th.
Exempt employees please note: Exempt employees participating in a furlough program become non-exempt for the week in which a furlough day occurs. Exempt employees must be specifically authorized to work more than 32 hours in a furlough week. No overtime has been authorized at this time.

Work Number For Everyone - New Verification of Employment Procedure
Beginning May 11, 2009, California State Employees (Faculty, Staff, and Students) working for CSUC may take advantage of a new automated system for handling employment and salary verifications called “The Work Number For Everyone”.
Whether you are renting an apartment, buying a house, purchasing a car, or doing anything else that requires employment and/or salary verification, The Work Number is available 24 hours a day, 7 days a week via touch-tone phone.
The benefits of this service are significant: potential quicker loan approvals, speedier employment verifications, up-to-date information, and round-the-clock access to employment and salary information for verifiers (organizations that need credit or employment reference information).

CAMPUS 4/10 SUMMER WORK SCHEDULE FOR JUNE 1-AUGUST 14, 2009

For the period of June 1 to August 14, 2009, excluding the week of June 29 - July 3, California State University, Chico will move to a 4/10 summer work schedule. The campus will be open Monday through Thursday from 7:00 a.m. through 5:30 p.m. daily with a half hour for lunch. The purpose of this new summer schedule is to provide an employee benefit of three-day weekends and reduce our carbon footprint. These hours will be the normal work schedule for most employees excluding those in 24/7 operations (such as the University Police Department).
Independence Day will be observed July 3, 2009. The work schedule for the week of June 29 through July 3, 2009, will be: Monday through Thursday 8:00 a.m. to 5:00 p.m., 8 hours each day, with one (1) hour for lunch. The campus will be closed on Friday to observe the holiday (8 hours).
Campus administration will make every effort to accommodate those staff employees for whom this schedule constitutes a hardship due to concerns related to childcare, elder care, medical issues, and physical limitations. So long as departmental needs are met, accommodations may be made through providing an alternative 10-hour work schedule (such as 6:00 a.m. to 4:30 p.m.) or through allowing use of CTO or vacation to reduce the hours of work for non-exempt employees. A Summer Hardship Exception Form will be provided on the Staff Human Resources Management (SHRM) web page for requesting review by SHRM of denied requests (http://www.csuchico.edu/hr/Forms/SHRM-SummerHardshipExceptionForm.doc). Those staff employees with an approved hardship schedule adjustment that do not have vacation time or CTO or do not want to use vacation to cover reduced hours shall use pay dock. Efforts will be made to provide opportunities to earn CTO prior to June 1, 2009, in order to minimize any loss of pay.
SHRM will provide two informational sessions, one for supervisors and one for attendance clerks, during the next few weeks. Dates and times for these sessions will be provided via campus announcements. Questions should be forwarded to Staff Human Resources Management at (530) 898-4666.

STATE EMPLOYEE FORM W-2

State Employee Form W-2
The State Controller’s office will have printed/mailed employee’s 2008 Form W-2 Wage and Tax Statement no later than January 16th. W–2's are mailed to your home address on file with the Campus Payroll Office. If a form is undeliverable by the U.S. Postal Service it should be returned to the Payroll Office, if you do not receive your Form W-2 please contact the Payroll Office at 898-6435.
In the event you lose your W–2, or require a duplicate copy for another reason, you can request one. You may obtain a Standard Form 436 (Request for Duplicate Wage and Tax Statement) from the Payroll Office (Kendall 222). The request must include your name, Social Security Number, the tax year(s) being requested, your mailing address, a daytime telephone number, and your authorizing signature. There is a $8.50 processing fee per tax year requested.
