Authorization Form:
The faculty member completes an authorization form, providing
the name, Chico State I.D. number, and barcode of the faculty
member and the designated student. The authorization form
will be provided by the Circulation Department and should
be returned there by either the faculty member or the designated student. The
authorization form will be kept on file in the Circulation Department.
Duration:
The authorization will be valid for the academic year, including
the summer, or, at the faculty member's request, for a period
shorter than the academic year. The authorization terminates immediately
should the designated student's status as currently
registered be discontinued.
Check Out:
The designated student should present the items requested by
the faculty member to the Circulation Department and inform staff
that permission is on file for the items to be charged to a faculty
member's record.
Due Dates:
Items charged to the faculty member's record will be due on the faculty due
date, April 15 th.
Recall:
Items are subject to normal recall procedures should they be requested
by another patron.
