Definition of the Degree-Major Program

1a. Name of the campus submitting the request.

1b. Name of the department(s), school(s), or other unit(s) of the campus which would offer the degree-major program.

1c. Name, title, and rank of the individual(s) primarily responsible for drafting the program.

1d. Objectives of the degree-major program.

1e. Total number of units required for major. List all existing courses to be specifically required for the major under the degree program. Identify those new courses which are (1) needed to initiate the program and (2) needed during the first two years after implementation. Include catalog description of all new courses.

1f. List of elective courses, by catalog number, title, and units of credit, which can be used to satisfy the requirements of the major. Identify those new courses which are (1) needed to initiate the program and (2) needed during the first two years after implementation. Include catalog description of all new courses.

1g. List of existing elective courses that can be used to satisfy requirements for the major.

1h. If any formal options, concentrations, or special emphases are planned under the major, explain fully.

1i. List of course prerequisites and other criteria for admission of students to the degree-major program, and for their continuation in it.

1j. Explanation of special characteristics of the degree-major program (e.g., in terminology, units of credit required, types of course work).

1k. Mode of delivery for the program's courses if other than regular (e.g., distance, cooperative, intercampus).

1l. For undergraduate programs, list of provisions for articulation of the major with community college programs.
