You may view various forms by clicking on the blue links below. Some of these forms are Adobe Acrobat PDF documents. On these you can directly type in the required information and then print a copy. To print a copy of a non-Acrobat form, after it appears on your web browser screen, either click on your web browser print button or save it, then open it, then click on the application print button.

Acknowledgement of Lab Safety Policies and Procedures 

Safety in our labs is of prime concern. Our Lab Safety Policies and Procedures are rigorously followed. All students, faculty, and staff using a lab must complete an Acknowledgement of Lab Safety Policies and Procedures form each year.

Petition to Take and Transfer Unarticulated Courses from Other Colleges

The Petition to Take and Transfer Unarticulated Courses from Other Colleges form is for current students planning to take and transfer unarticulated courses from other colleges/universities to meet major course requirements for a Bachelor’s degree in Mechanical Engineering, Mechatronic Engineering, or Sustainable Manufacturing at CSU, Chico.  Unarticulated courses taken without prior approval will not be accepted for course substitution within the Department of Mechanical and Mechatronic Engineering and Sustainable Manufacturing.

Prerequisite Certification and Request to Enroll Student

If you have not taken a prerequisite for a course, but have taken a course which is equivalent to the prerequisite, have your major advisor sign a Prerequisite Certification and Request to Enroll Student form and submit it to Ms. Layne in the Department Office who will manually enroll you.

Request for an Exception to a Requirement 

If you would like to receive approval for an exception to a Department policy or graduation requirement such as a course substitution, you should request approval on a Request for Exception to a Requirement form.

SMFG 454 - Advanced Laboratory Practices 

You must obtain faculty approval before registering for SMFG 454 by submitting a SMFG 454 Request Form.

To Add a Course

To add a course after the second week of classes, a Class Add/Update Request form must be completed including appropriate approval signatures.

To Drop a Course

To drop a course after the second week of classes, a Class Drop Request form must be completed including appropriate approval signatures.
