RNs: Apply for a Public Health Nurse Certificate

To be certified as a Public Health Nurse, you must be in possession of a valid California Public Health Nurse Certificate issued by the Board of Registered Nursing
.

1. Request Official CSU, Chico Transcripts.

Go to the CSU, Chico Records Office "transcript request" link http://www.csuchico.edu/sro/forms/transcript.shtml and order an official transcript to be sent to yourself.  DO NOT OPEN.

2. Complete BRN Application for California Public Health Nurse Certificate and submit to the BRN.

A. Complete the application.  Click this link Application and scroll down to the form. 
  • For the section regarding verification of child abuse/neglect prevention training, fill in the following:
  • CE Provider/School name:  CSU, Chico.
  • Course Name and Number: Include the following statement, “The content of CSU, Chico curriculum includes the child abuse/neglect prevention training.”
  • Number of Hours:  Not needed.  Leave blank.
B. Attach $75 fee with unopened official CSUC transcript, and mail to:

3. Complete BRN Request for Transcript/Public Health Nurse Certification form.Click this link Application and scroll down to the form. 

Mail this BRN form to the School of Nursing office at the following address.  This form will be completed by the nursing office and submitted directly to the BRN to be matched with your application.
