Thank you for your interest in the Rural California Nursing Preceptorship Program.  We suggest you begin by reading a brief Course Overview that we have provided to better acquaint you with the program. 

APPLICATION CONTENT
The following items comprise your completed application:

• Two Statements of Recommendation (must be completed no earlier than 3 months prior to the Application Deadline)

• Letter of Introduction

• Skills Proficiency Check List

• Goals and Objectives

• Required Medical Records (seasonal flu vaccine records not due with application)
• Declination Form for Flu Vaccine, optional
• Declination Form for Hepatitis B Vaccine, optional
  
• Personal Photo
• Record of Personal Health Insurance - submit proof of coverage, e.g. copy of insurance card
• Copy of RN license (if currently a registered nurse)
• Copy of current (CPR) BLS for Health Care Professionals card
• Housing Questionnaire - optional, only required if you need housing
• $100 Application Fee (check or money order only)
• Application Form (not available for download until 8/21/13)
• Application Checklist - for first-time participants

APPLICATION INSTRUCTIONS
Please follow the specific instructions given for each of the above items.  APPLICATIONS ARE NOT CONSIDERED COMPLETE AND WILL NOT BE ACCEPTED WITHOUT THE WINTER 2014 APPLICATION FORM. 

APPLICATION CLOSING DATE
The application period for Winter 2014 will close on 9/6/13, i.e. all applications must be postmarked by this date in order to be considered.

SUBMITTING YOUR APPLICATION
All application materials must be submitted either by mail or be hand-delivered.  The only exception will be Statements of Recommendation and the digital photo which are to be sent to the RCNP Coordinator via email as instructed.

PRIORITY APPLICANTS
50% of placements are reserved for current CSU, Chico, Butte College, and Shasta College students.  However, these students are subject to the requirements and deadlines outlined above.  The remaining slots are filled with students from other colleges and universities, RN Refresher students, and RN graduates.   Applicants are selected based on their 1) completed applications, 2) references, and 3) recent or previous breadth of experience in the acute clinical setting.

CSU, CHICO REFRESHER STUDENTS
Except in special circumstances, CSU, Chico nursing refresher students must complete their Refresher Course before they are eligible to apply for the RCNP Program.
