Competitive Club Rosters and Recognition

  • Organization Recognition Form (pdf)–this form is due at the beginning of each semester in order to continue club recognition for both competitive and recreational clubs.
  • Competitive Sport Club Handbook (pdf)–use this for reference for everything from membership to facilities reservations.
  • Officer Declaration Page (pdf)–competitive club teams must declare their president, vice president, treasurer, safety officer, and trip leader on this form each semester and submit to the Sport Club office PRIOR to starting organized activity.
  • Individual Membership Application (pdf)–individual athletes must download this packet, complete, and return to club president. This is the only way to be added to a competitive club roster.
  • Funding and Re-Recognition Application (doc)–club officers must download this application and submit by April 24 to apply for recognition and funding as a Competitive Sport Club.
  • Sport Club Constitution - all recognized clubs are required to file a current constitution with the Sport Club office. This link takes you to a boilerplate model that you may use to develop your club constitution.
  • Special Event Waiver (doc)–use this waiver for non-club participants, alumni, or special guests traveling with the team (ie. parents, spouses, children) or for any non-students participating in club events. If participant or traveler is under the age of 18, then consent must be granted by parent or guardian.

Recreational Club Rosters and Recognition

  • Organization Recognition Form (pdf)–this form is due at the beginning of each semester in order to continue club recognition for both competitive and recreational clubs.
  • Sport Club Constitution - all recognized clubs are required to file a current constitution with the Sport Club office. This link takes you to a boilerplate model that you may use to develop your club constitution.
  • Rec Club Team Roster (pdf)–recreational club officers must download this roster sheet to submit to the Sport Club office to record membership for Recreational Clubs level I and II.
  • Rec Club Officer Code of Conduct Contract (pdf)–all recreational club presidents and treasurers must have a signed copy of this on file in order to be recognized as a club.
  • Rec Club Waiver (doc)–individual recreational club members must complete this waiver if directed for participation on a Recreational Club.
  • Rec Club Insurance and Pre-existing conditions Form (pdf) - this is needed by designated clubs only. The Recreational Club Advisor will notify the club president if this is needed by each member of the club.
  • Volunteer Coach Employee Form (pdf)– all club leaders and volunteers assistant coaches must submit this form annually to the Rec Club Advisor.

Finance

  • Budget Plan Worksheet (xls)–Club treasurer should download this template and work with other officers to create a budget plan. Plan must be submitted in this form for approval before accounts can be accessed for the year.
  • Expense Request (pdf)–Club treasurer must submit this form along with receipts/invoices for purchases, member reimbursements or travel.
  • Multiple Traveler Form (doc)–Use this form to authorize a single individual to be reimbursed for expenses incurred by multiple travelers.
  • Player Dues Record (xls)–Club treasurers will download this spreadsheet and keep track of dues paid by club members throughout the year. When dues are submitted to the Sport Club office by the club treasurer, he/she will note the date and apply to each individual record. Form will be updated electronically AND on paper in the club treasurer binder.
  • Training Kit Supply Order Form (xls)–Safety officers must download and complete this form and submit to the Sport Club office to re-order REQUIRED equipment for the club training kit.
  • Vendor Contract (pdf)–you must use this form in order to have any outside vendors (food or merchandise) on campus at your home event.

Travel

  • Club Driver Packet (pdf)–Club members must download and complete this five-page packet in order to become an eligible driver for their club on roadtrips.
  • Transportation Manifest (pdf)–Trip Leaders must submit this manifest at the pre-trip meeting prior to travel declaring all trip drivers and passenger assignments.
  • Post Trip Report (pdf)–Trip leaders must submit this report AFTER the trip and BEFORE the club is approved to go on the next trip.
  • Travel Excuse Letter (pdf)–any current sport club athlete may download this letter to present to professors prior to approved team travel.

Coaches/Officials/Leaders

  • Paid Coach Responsibility Form (pdf)–download a copy of this contract to officially offer a coaching position. Must be signed by both coach and club president and submitted to the Sport Club office for approval. Separate payroll paperwork must be completed based on student or non-student status.
  • Volunteer Coach Employee Form (pdf)– all club leaders and volunteers assistant coaches must submit this form annually to the Sport Club office.
  • Officials Payroll Packet (pdf) - this packet must be completed PRIOR to officiating and submitted to the Sport Club office AFTER officiating. All officials checks will be mailed within 10 working days.
