11.30.2005 -Payee Data Record now required

Certain payments may now be subject to Federal income tax withholding - Any one of the following types of payments made from the Research Foundation or University Foundation may be subject to Federal income tax withholding:

Fees for service, scholarships, honoraria, room and board, stipends, living allowances, transportation, royalties, prizes and awards.

IRS regulations may require income tax withholding for recipients of these types of payments who are not U.S. residents. As a result, beginning immediately, anyone receiving these types of payments will be required to complete a Payee Data Record form before a payment will be processed. We need your assistance by having a completed Payee Data Record attached to check requests that are for these types of payments. A Payee Data Record form can be located and printed from: http://www.csuchico.edu/resp/form/payeedatarecord.doc.
