01.23.08 New Personnel Action Forms and New Hire Procedures

RE: New Personnel Action Forms and New Hire Procedures

NEW PAFS
We are pleased to announce that the CSU Chico Research Foundation Personnel Action Form (PAF) and Administrative Data Form are now available on the Foundation website: http://www.csuchico.edu/rfdn/Forms.htm These forms can be completed on-line and printed or printed and completed by hand. Simply complete the form, sign it, and route as you have done previously: either directly to the Foundation Administration Office (zip 246), or the Office of Research and Sponsored Programs (zip 870). You may keep a copy if you choose, but at the end of the approval process, the Foundation will make a copy for your project files and will also forward one to the employee.

The forms have a slightly different look. We have combined the old cycle 1 & 2 (black & red) PAFs and a check box will be used to designate the type of employee (benefited or non-benefited). As part of the PAF, a second page will be printed entitled “Conditions of Employment.” Since it is no longer possible to print that information directly on the back of a PAF, it will need to be printed, signed and stapled to all PAFs requiring employee signatures.

NEW PROCEDURES
Starting immediately we will also have some new procedural changes in the processing of new hires onto Foundation payroll. While these changes are being strongly driven by some compliance and documentation issues, we believe they will ultimately relieve you and your staff from some of the administrative processes associated with a new hire.
• The new hire PAF, Administrative Data Form must be downloaded and completed by the project director/supervisor and employee. In addition, for cycle 2 non-benefited employees, the Foundation application form must also be downloaded and completed by the new employee.
• All new hires onto Foundation payroll (or re-hires) will be required to come to the Foundation Administration Office (25 Main Street, Ste 203) for the completion of the new hire packets and a brief orientation. They should bring the completed & signed PAF, Administrative Data Form and application with them. The HR office will be available for orientations at 9 am, Monday, Wednesday & Friday and by appointment at other times. If you have employees who work and live out of the area, please contact the HR office for special arrangements.
• In addition, new employees will be required to bring original proof of their eligibility to work in the United States (I-9 completion documentation—a list of acceptable documents is found at our website) as well as their social security card for W-4 verification purposes.
• New employees should plan on spending approximately 20 minutes completing their paperwork and receiving their orientation.
• Non-Resident Alien student employees will require an appointment for the completion of their paperwork and will need to bring their Passport, VISA, I-94, I-20 or DS 2019 and social security card along with them to their appointment.
