Independent Contractor Agreement
The Independent Contractor Agreement is an agreement between the independent contractor and the CSU, Chico Research or University Foundation (Foundation). The agreement includes terms and conditions and a Payee Data Record form. The agreement and all applicable forms must be completed, signed by the appropriate person and approved and signed by an authorized Foundation representative prior to the start of any services by the IC.
