Independent Contract Agreement

The Independent Contractor (IC) Agreement contains the three parts listed below. Part I and Part II are required for every agreement. Page one of Part I is the instruction sheet for preparing the agreement. The Project Director Responsibilities section, on the instruction page, details when Part III is required. The first page of Part II contains instructions for the IC. If you have questions about preparation of the agreement, contact the appropriate Foundation office.
