Workers' Compensation Reporting Procedure
Employees' Claim Form (Must be obtained from either your supervisor or the Research Foundation Human Resources office, 25 Main St., Ste. 203).
Safety Hazard Report Form (Please use this form anytime you encounter a safety hazard to avoid future accidents)
