Health History Form
To locate the Health History form follow the steps below...
- Click the Health History Form link (above).
- Log into the Health Center Portal using your student username & password.
- Click 'Forms' on the left menu, then select 'Health History Form'
Appointments
For your convenience, you may schedule an appointment with a medical professional or enroll for Family PACT online, review your scheduled appointment, reschedule, and/or cancel your appointment. This service is available 24 hours a day/7 days a week.
ATTENTION:THERE IS A $10.00 to $25.00 CHARGE FOR EACH MISSED APPOINTMENT DEPENDING ON THE REASON FOR VISIT
Questionnaires
If you were directed to fill out an online questionnaire by a Student Health Center staff member, this is the link to select a secure portal. Completed questionnaires become a part of your medical record and are strictly confidential. This service is available 24 hours a day/7 days a week.
Secure Messaging
If you would like to send a secure message to a provider you have seen recently at the Student Health Center, choose this link. Please note, if you do not see an option to send a message to the clinician of your choice, please call 530-898-5241. This service is available 24 hours a day/7 days a week.
Pharmacy Prescription Renewal
If you have a prescription (with refills) on file at the Student Health Center Pharmacy, you may request a refill online by choosing this link to take you to a secure messaging center. This service is available 24 hours a day/7 days a week.
