If you would like to obtain a copy of your medical record for your own personal use, there is a fee of $5.00 for the first 25 pages and then .15 for each additional page. Please call 530-898-5241, option 0 to speak with a Health Information Department staff member regarding the exact fee for a copy of your personal medical file. You will need to complete a Medical release form and submit a check to CSUC for the agreed payment amount (either by mail or in person) before your record can be released to you.
In order for Student Health Service to ensure maximum utilization of clinic access for students, the following no-show policy will be adhered to:
- No-show for scheduled specialty visit with psychiatrist or orthopedist, $25 charge to patient.
- No-show for scheduled specialty visit for colposcopy, $25 charge to patient.
- No-show for scheduled primary care appointment, $10 charge to patient.
Students will be informed at the time they schedule their appointment that no-show fees apply (either via the web or phone call). Students will also be sent an e-mail reminder with information regarding the scheduled appointment and no-show fees approximately 24 to 48 hours before the appointment.
A missed appointment will be considered a “no-show” once the appointment time has passed. Medical Records staff will mark missed appointments as a “no-show” and send the patient a secure message indicating the amount of the charge to their account.
