BFA Application Forms and Materials Checklist

Please prepare a file folder, with your name on it, with the following inside:

• BFA Portfolio Review Prerequisite/Application Form (PDF) (PDF), including Art GPA
• Major Clearance/Advising Form (PDF)
• Unofficial Transcriptions from:
  • California State University, Chico
  • Any other universities attended
  • Any junior colleges attended
• A one-page written personal statement about your artistic/professional goals and creative work
• Personal statement emailed to BFA coordinator

You must also provide two (2) faculty members with the Faculty BFA Recommendation Form (PDF). You must complete the first section. The faculty will give the completed forms directly to the BFA coordinator.
