FFY 21 Update
- Civil Rights
- All staff working on the project should complete Civil Rights training during onboarding, and then annually
- Confidentiality
- All staff currently doing application assistance, who may do application assistance, or are handling confidential data should complete Confidentiality training during onboarding and then annually
- Project coordinators/directors are required to document that their staff has completed the required training on the CalFresh Outreach Confidentiality & Civil Rights Tracker (listed below) under Documents Needed for Tracking and keep it on file
- Project coordinators/directors must confirm with CHC that your program is in compliance with the steps listed above by completing the Confirmation Form (listed below) under Documents Needed for Tracking, no later than April 15, 2021

- Confirmation Form
- Only needs to be submitted to the Center for Healthy Communities once for the whole contract period.

- This can be done as a team or individually – please input “First Name” as first and last name and input “Last Name” as your campus or organization when signing up. If done as a team, please use program coordinator’s name.
