COVID-19 Vaccine Self-Certification Process for Students

In accordance with the pending CSU policy, make sure you are fully vaccinated against COVID-19 before the start of the semester. The COVID-19 vaccine is safe, effective, and the best way you can protect yourself and others. 

As an important step in safely reopening, Chico State is now ready to collect and verify vaccine information from students. As of today, students can voluntarily submit their vaccination status via the Student Center by clicking “Covid 19 Vaccination” in the Student Records menu. 

Using the form, you will indicate whether: 

  • you have been vaccinated and upload a picture of your vaccine record card 
  • you qualify for a medical or religious exemption 
  • you do not plan to access any campus facilities or participate in any in-person activities 

View detailed instructions on how to submit your vaccination status. 

We encourage all students to complete the form as soon as they are fully vaccinated (or know they plan to pursue an exemption) so we can begin verification.  

While this process is currently voluntary, you will be a step ahead when the FDA grants full approval and vaccines are required. Once you complete the Student Center form, you have fulfilled the requirement and will not have to submit vaccination records again. Additionally, pending University and CSU systemwide policy, fully vaccinated students who have submitted their information may soon be able to remove face coverings when indoors and be exempt from future COVID-19 surveillance testing, should the need arise for it in the future.  

Exemptions  

Please indicate on the Student Center form if you are seeking a medical or religious exemption to the upcoming immunization requirement and be aware of these additional steps:   

  • If you are requesting a medical exemption, you must also fill out the Accessibility Resource Center’s registration form and provide documentation verifying the medical issue.  
  • If you are requesting a religious exemption, once you have filled out the Student Center form, you will be contacted by the Title IX/DHR office and provided with information on how to submit your request for religious exemption through an online form.   

Please note, students who choose to live in University Housing or participate in NCAA athletics are required to demonstrate proof of being fully vaccinated (e.g. uploading their vaccine record card).
