Workers' Compensation
Chico State Enterprises utilizes California State University Risk Management Authority (CSURMA) along with claim administrator Sedgwick CMS as our Workers' Compensation Insurance partners.
For information on reporting procedures, including treating facilities, see the documents below. In the event of an injury or illness, please be aware of the importance of reporting guidelines.
The following are to be completed by the Supervisor of the injured employee:
- Accident Investigation Report (PDF)
- Form 5020 - Employer’s Report of Occupational Injury or Illness (PDF)
- DWC-1 - State of California Workers' Compensation Form (PDF) - Do not complete unless the employee is seen by a medical provider for a work-related injury or illness
