Guidelines for Name Changes

Department or Unit Name Change

A name change to a college, department, center, or similar major unit may be initiated by the unit or the Provost's Office. Please submit the Department or Unit Name Change Request(opens in new window).

  • Click on Propose New Miscellaneous then choose College, Department, or Unit Name Change from the Request Type dropdown menu. For help with the form, or to request access, please contact Curriculum Services.

The proposal will be submitted by Curriculum Services to the Academic Senate via EPPC for review and recommendation. The Academic Senate will submit its recommendation to the Provost who will, in turn, review and make a recommendation to the President. If approved at all levels, the name change will become effective with the issuance of an Executive Memorandum.

Undergraduate or Graduate Program Name Change

A program name change may be initiated by the department under which the program resides or by the Provost's Office. Please submit the request using the online Program Form(opens in new window)

The proposal will be submitted by Curriculum Services to the Academic Senate via EPPC for review and recommendation. The Academic Senate will submit its recommendation to the Provost who will, in turn, review and make a recommendation to the President. 

If the name change requested is for a degree program, Curriculum Services will request approval from the Chancellor's Office prior to campus action. For Chancellor's Office proposal requirements please read Changing A Degree Title or Suggesting a New Code. (PDF)   

An Executive Memorandum will be issued when the name change has been reviewed and approved at all necessary levels.

Note: A program which undergoes a name change will continue reporting using the reporting code assigned under the old name unless otherwise stated on the Executive Memorandum.
