Immunization Requirements

California State University (CSU) requires all students to be current on a number of immunizations and to complete a risk assessment for tuberculosis (CSU Executive Order 803).

This executive order applies to undergraduate and graduate students, students matriculated in self-support degree programs and K-12 students participating in concurrent enrollment courses entering the California State University (CSU) in or after fall 2023.

Immunization Compliance Deadlines

Currently Enrolled Students: March 1, 2023

If required documentation is not received, a registration hold will be issued which will prevent registering for classes for the following semester.

New or Transfer Students: 1st Day of Enrolled Semester

Required Immunizations

All new and incoming students who will begin classes in the fall 2023 semester must submit proof that they comply with the following immunization requirements. This applies to incoming international students and graduate students as well. 

Required Immunizations & ScreeningsDosage and Screening Information 
Hepatitis B (Hep B)

Required for all students who are 18 years old or younger

Measles, Mumps & Rubella Immunization 

Required for all students who were born after January 1, 1957

  • Two (2) doses are required on or after the patient's first birthday

                   OR

  • A positive titer 
Varicella (Chickenpox)

Please note that prior history of contracting the disease does not meet compliance for EO 803

  • Two (2) doses are required on or after the patient's first birthday 

       OR

  • A positive titer 
Tetanus, Diphtheria and Pertussis (Tdap)

One (1) dose is required after the age of 7 

Meningococcal Disease (Serogroups A, C, Y, W-135)

Required for all students who are age 21 or younger 

  • One (1) dose is required on or after the age of 16
Tuberculosis Screening/Risk Assessment (TB)

All incoming students are required to complete a Tuberculosis (TB) risk questionnaire. Students who are at a higher risk for contracting a TB infection should undergo additional testing within 1 year of entering CSU, Chico. Please contact the WellCat Health Center for more information. 

You can complete the TB screening questionnaire under the Medical Clearances tab in the Patient Portal.

Incoming CSU students are strongly encouraged, but not required, to receive the following immunizations. We encourage students to discuss these with your provider.

How do I upload my immunization record?

To have their immunization record verified by the WellCat Health Center, students need to upload an image or scan into their patient portal. Students can find the upload link under the "Medical Clearances" tab. You can follow the steps demonstrated in this video: How to Upload Your Immunization Record
