NEW and IMPROVED MPP Performance Evaluation form - available now!
In collaboration with Cabinet, the Human Resources Services Center team redesigned the form to better support administrators and reflect the expectations of campus leadership. The updated version includes:
- Delegation option: Administrator can now delegate the responsibility of initiating the evaluation.
- Pre-filled employee information: Just enter the employee’s Chico State ID, and key detail will auto-populate.
- Streamlined workflow: The form automatically moves from administrator, to employee, to Human Resources for filing.
- Updated rating criteria: Clear definitions help guide evaluations.
- Refreshed Performance Criteria: Better aligned with the expectations of campus leadership.

I highly recommend that you use the step-by-step instructions the first time you use this new tool. When initiating the process with your MPP, please consider sending them a link to the step-by-step instructions for employees so they understand how to navigate the new form (also found on this website).
If you’ve already started drafting 2024-2025 evaluations for MPP on your team using the “old” form, you do not need to start over. Links to the “old” form will stay active and supported through September 2025.
