Public Service Loan Forgiveness (PSLF) - Employment Certification

The PSLF Program forgives the remaining balance on your Direct Loans after you have made 120 qualifying monthly payments under a qualifying repayment plan while working full-time for a qualifying employer. Under this program, CSU employees may qualify for forgiveness of the remaining balance due to their Direct Loan Program loans if they meet the criteria of the program.

Employment Certification

HR will complete Sections 3 and 4 to verify employment is with a public service organization. Original Employment Certification forms must be submitted to our office. Completed forms will be returned to the employee.

It is the responsibility of the PSLF applicant to submit the application, and all required paperwork, to the U.S. Department of Education in order to qualify for the PSLF Program. The U.S. Department of Education, not the CSU, reviews the applications and makes the determination of eligibility for the PSLF Program.

Tips for form completion:

  • Employer Name: California State University, Chico
  • Employer's EIN/FEIN: email hrsc-front-office@csuchico.edu for the campus' unique EIN for this program. 

Email Process

  1. Download the form from the appropriate website. Rather than using the PSLF Tool, instead, complete the form on your computer without the SSN and save it as a PDF file. (You may need to "print" to Adobe PDF or Microsoft Print to PDF.)
  2. Email the PDF to: hrsc-front-office@csuchico.edu 
    1. Subject: PSLF Employment Certification Form
    2. Attach the form

Upon HR’s receipt, your form will be processed accordingly. Signed forms will be returned electronically via Adobe Sign.

Current CSU, Chico employees: 

You will receive an email from adobesign@adobesign.com notifying you the document has been signed. Log in to Adobe Sign and navigate to your completed agreements folder. Download a copy of your completed application. Ensure the form has your SSN and signature prior to submitting it to the requesting agency. 

Previous employees:

You will receive an email from adobesign@adobesign.com notifying you the document has been signed. Click on the link provided. You will be prompted to enter a password. The password will be the last six digits of your SSN.

USPS Process and In-Person 

Mail or submit pages 1-2 of the application to KNDL 220. Upon HR’s receipt, your form will be processed accordingly. Signed forms will be returned electronically via Adobe Sign adobesign@adobesign.com. Please refer to the appropriate section above to learn how you will receive and access your completed form.
