Forms and Resources

Wildcat email

All email communication (class information, add/drops, advising, degree progress report, graduation, etc) go through your Wildcat email.

When sending an add, drop, plan change request, include:
  • Student ID number
  • Course name and section
  • Registration number (four-digit)
  • Supporting documents if needed

Student Records & Registration requires all documentation to be submitted through your Wildcat Email account(@mail.csuchico.edu).

Use your Wildcat email when submitting your forms to your Instructor and the
MUTA office. All editable forms need to be saved as PDF before sending. Read Convert your form to PDF below.
Convert your form to PDF or Flattened Form
Fillable forms automatically reset and recipients receive a blank form even though you filled it out. A flattened form is a fillable form that has been completed then converted to a PDF. The form is no longer fillable and your information is saved. 
 
Follow these steps:
  • Complete the form and save your information by selecting
    • DOWNLOAD SAVE AS PDF or 
    • PRINT TO PDF 
      • File name: YOUR LAST NAME, FIRST NAME
    • Save your PDF on your desktop, in downloads, or where it is convenient
  • Email your PDF to your instructor or department advisor 
    • Double check your attachment before you hit SEND
  • Notice after a certain date there may be a late fee
