Registrar Forms
MUTA Forms
Convert your form to PDF or Flattened Form
Fillable forms automatically reset and recipients receive a blank form even though you filled it out. A flattened form is a fillable form that has been completed then converted to a PDF. The form is no longer fillable and your information is saved.
Follow these steps:
  • Complete the form and save your information by selecting
      • File name: YOUR LAST NAME, FIRST NAME
    • Save your PDF on your desktop, in downloads, or where it is convenient
  • Email your PDF to your instructor or department advisor
    • Double check your attachment before you hit SEND
  • Notice after a certain date there may be a late fee
Have a successful virtual semester

Standard procedures are being modified to accommodate a virtual semester. There will be a great deal of Wildcat email communication throughout the semester. Places to check for class information on a regular basis:

  • Wildcat email (, junk mail, trash bin
  • If you use your personal email address (, hotmail, gmail, etc) you may be asked to re-submit documentations and approvals.
  • platforms your instructor has chosen to use for your class
  • This page will be updated throughout the semester.

If the answer to your question is not here, you can email your instructor or the MUTA office.

Synchronous vs Asynchronous

  • Online Synchronous - Meet in real time at the scheduled meeting days and times.
  • Online Asynchronous - No scheduled meeting days or times, but the student is responsible for completing course work; refer to syllabus.
  • Reach out to the Student Learning Center for academic support.
