About Family PACT

What is Family PACT?  The Family PACT program offers low cost reproductive health services to low income California residents.  

How do I know if I am eligible?  You are eligible if you currently live in California, are of reproductive age, are at or below 200% of the Federal Poverty Level (PDF), and have no other source of health care coverage for confidential family planning services. If you are confused as to whether or not you are eligible, check out this Flow Chart (PDF) that explains the requirements step by step. Come into the SHS or call (530)-898-4460 if you have any questions and would like to talk with Family PACT enroller directly.

How do I Enroll?  Enrollment is a quick process which requires you to make an appointment, fill out a CEC form (PDF) and meet with a Family PACT enroller.  These appointments are 15 minutes and are meant to give you an opportunity to ask any questions you might have about Family PACT.   If you prefer, you can download, print and fill out the CEC form now using these 15 Quick Tips (PDF) to assist you (in case the form becomes confusing).  If you come in for your appointment without filling out the form, then it will be given to you when you check in for your appointment and can be completed in the waiting room. 

What if I already have a card?  If your card is active (you received it within the last year), then you can notify the receptionist and receive free services.  If you are unsure about whether your card is active, we encourage you to make an appointment to check the status. If it isn’t active, an enroller will simply recertify your card on the same day!
