Forms
To locate the forms follow the steps:
- Log into the Health Center Portal using your student username & password
- Click “Forms” on the left menu, complete all forms, being sure to click "submit" after each one
Family PACT
For your convenience, you may schedule an appointment for Family PACT online.
Questionnaires
If you were asked to fill out an online questionnaire by a WellCat Health Center staff member, click the link above. Completed questionnaires become a part of your medical record and are strictly confidential. This service is available 24 hours a day, 7 days a week.
Secure Messaging
If you would like to send a secure message to a provider you have seen recently at the WellCat Health Center, choose this link. Please note, if you do not see an option to send a message to the clinician of your choice, please call 530-898-5241. This service is available 24 hours a day, 7 days a week.
Prescription Renewal
If you have a prescription (with refills) on file at the WellCat Health Center Pharmacy, you may request a refill online by choosing this link(opens in new window) to take . This service is available 24 hours a day/7 days a week.
