Health Information (Medical Records)
The Health Information Department of the Student Health Service retains your medical records for up to ten years after your last date of visit. All medical records are stored in a secure location on site.
Medical Record Copies for Healthcare Provider
If you would like to obtain a copy of your medical record for another health provider, please complete the release form and mail it to 400 W 1st Ave, 95929-0777 or fax it to our secure line (530-898-4057). Be sure to complete each section of the form, including the healthcare provider's name, address, phone and fax number. Health Information will send the copy of your medical records to your provider in the most appropriate manner consistent with the size of your records and the timeliness of the request. Medical Record Copies for Personal Use
If you would like to obtain a copy of your medical record for your own personal use, there is a fee of $5.00 for the first 25 pages and then .15 for each additional page. Please call 530-898-5241, option 0 to speak with a Health Information Department staff member regarding the exact fee for a copy of your personal medical file. You will need to complete a release form and submit a check to CSUC for the agreed payment amount (either by mail or in person) before your record can be released to you.
