Job Description: 

The team of social workers at the Peg Taylor Center assesses and addresses the needs of program participants to support their overall health, safety, independence, and well-being. Our social workers provide information to participants and their families about the program, conduct initial and ongoing assessments, design person-centered care plans to improve and maintain psycho-social wellbeing, and provide social work support and treatments. They also provide oversight and assistance with related services provided to the Peg Taylor Center’s participants by our Program Assistants and other staff and social work interns. The Social Worker working part-time is teamed with other Peg Taylor Center Social Workers to provide a high quality of services. The regular schedule for this part-time position is anticipated to be 25 hours/week, M-F. Compensation depends upon experience. Click on the link for more information.

Required experience, skills and education include OIG clearance if social worker is licensed, as well as a demonstrated understanding of the psycho-social needs of older and disabled adults and the ability to design effective care plans to improve the well-being of program participants over time. The successful candidate should be able to communicate positively and effectively with participants, their families and other professionals, work well in a team environment, and provide effective supervision of participant care. Prior experience of at least 1-3 years in a similar setting is preferred. Exemplary references are a must

Job Description: This is the full professional journey level class within the HHSA Mental Health Counselor, Senior Series. Employees within this class are distinguished from the HHSA Mental Health Counselor, Senior I by the performance of the full range of duties as assigned, requiring attainment of a Licensed Clinical Social Worker or Marriage Family Therapist License. Incumbents must possess a significant level of specialized, technical expertise beyond that expected at the entry level. Positions at this level require highly specialized knowledge, skills, abilities and experience, and often exercise independent judgment in the performance of their duties. Positions in this class are flexibly staffed and are normally filled by advancement from the entry level, or when filled from the outside, have prior experience or required licensing. The HHSA Mental Health Counselor, Senior series is distinguished from the Mental Health Counselor series in that the former provides technical-level group and individual case and social services referral work.

Receives general supervision from higher-level supervisory or management staff; may provide leadership and coordination of activities of lower-level staff as assigned

Job Description: This is the professional entry level class in the HHSA
Mental Health Counselor, Senior Series. This class is distinguished from the HHSA Mental Health
Counselor, Senior II by the performance of the more routine tasks and duties assigned to positions
within the series and by the lack of a required license. Since this class is typically used as a training
class, employees may have only limited directly related work experience. As a training class, this class
is limited to six years in assignment before the appropriate professional licensure is required (Article 1,
Regulation 4996.18 of the Business and Professional Code of California).

Receives general supervision from higher-level supervisory or management staff.


Knowledge of:
Pertinent federal, state, and local laws, codes and regulations.

Principles, practices, methods, and techniques of psychiatric care, including assessment, evaluation, and treatment methods.

Individual, family, and group psychological, sociological, and environmental problems encountered in conducting a public mental health program.

Ability to:
Interpret and apply federal, state, and local policies, procedures, laws, and regulations.

Interview and counsel clients, diagnose needed services and take effective courses of action.

Medical Social Worker

Job Description: Responsible for the delivery of social work services including initial psychosocial assessments, ongoing counseling, direct casework services, bereavement services, community education, outreach and referral.

· Performs psychosocial assessments, ongoing reassessments, counseling/education, crisis intervention and referrals for patients/families as required

· Documentation is completed in the patient’s electronic medical record (EMR)

· Actively participates in coordination of all aspects of patient’s care, in accordance with current professional standards and practice

· Participates/contributes in ongoing interdisciplinary comprehensive assessment

· Develops and evaluates the Plan of Care (POC) and documents in the EMR

· Serves as an advocate for the patient/family unit, and assists them with administrative, regulatory and financial procedures/paperwork including final arrangements

· Serves on-call duties and maintains caseload as assigned by CSPM or the Director of Patient Care Services. Caseloads may vary based on management’s evaluation of patient/family and HOF needs


Knowledge and Skills

· Ability to use independent judgement to make sound decisions

· Maintain confidentiality and follow HIPAA and patient privacy laws

· Strong, clerical skills including tying, copying, scanning and faxing

· Critical thinking and provide necessary interventions

· Communicate effectively, both orally and written

Education & Experience


· Two years’ experience in family and individual counseling and crisis intervention or home health social work
