Action Research Reporting

Reporting Instructions


The purposes for action research reporting are (1) to help you, the teacher, manage and reflect on your progress toward determining the effectiveness of an intervention and making improvements, and (2) to provide us with information that will help us develop more effective tools and processes for instructional improvement.


Each team member is responsible for reporting on the action research they are doing in their individual classroom. Step 1 will typically look very similar, if not identical for  for all team members. However, survey responses for the remainder of e cycle (steps 2 – 5) may be slightly different depending on variations in the intervention implemented in the classroom by different teachers. 

Action Research Surveys steps 1-5:

  • Survey Steps 1-5 will be unlocked starting the first of the month in which the survey is due and closed on the due date.
  • If you work on more than one step of the action research cycle during the reporting period (even if the step is not totally complete), please submit a separate survey for each step.

Professional Development Survey:

  • Any professional learning that takes place during the reporting period can be submitted anytime during the reporting period.

Answers to Commonly Asked Questions

Q. How do I know which action research survey to use?
A. Key components are listed on the Teachers’ PL- INC action research model. Compare these action items with what you are doing and select the one that is the best fit.

Q. Suppose I am in the same phase of the action research as the last time I submitted an action research survey, do I still need to submit a new one?

A. We realize that some phases in the action research are more time intensive then others. However, you need to complete an action research survey each reporting period even if you are in the same step. 

Q. During the reporting period I implemented multiple steps in the action research cycle, which action research template do I use?
A. If multiple step of the action research cycle have been implemented then you will need to submit multiple forms. The depth of information may vary but each phase of the action research cycle should be documented.
