New COVID-19 Self-Reporting Form

Students, faculty, and staff who test positive for COVID-19 should now report their case via our new online reporting form(opens in new window). The self-reporting platform replaces the COVID-19 hotline, which will close on February 28. Only COVID-19 positive tests need to be reported (not symptoms or exposures).

In follow-up to the PMT’s February 10 campus-wide message, students, faculty, and staff who test positive for COVID-19 should now report their case via our new online reporting form(opens in new window). Upon submission of the details of your case, you will receive an email with instructions for isolating. 

The self-reporting platform replaces the COVID-19 hotline, which will close on February 28. Only COVID-19 positive tests need to be reported (not symptoms or exposures).  Information about positive cases will now be posted on our COVID-19 website, in alignment with Cal/OSHA guidelines. The campus community can view information (date & location) about current cases to check if they may have been exposed.

Those who come down with symptoms should self-isolate and test as soon as possible. If you test negative, CDPH recommends isolating while you have symptoms and repeating testing every 1–2 days for until your symptoms resolve.

Asymptomatic individuals who have been exposed to COVID-19 do not need to quarantine but should test 3-5 days from last exposure, monitor symptoms and wear a mask for 10 days.
