Human Subjects Forms
In most cases research involving the use of human subjects must be approved by the University's Human Subjects in Research Committee (HSRC). Forms and application information are listed below. Questions and requests for assistance should be directed to Diane Smith, HSRC assistant at 898-4766, Student Services Center (SSC), Room 440 or John Mahoney, Ph.D. Chair, HSRC, at jmahoney@csuchico.edu.
Note: If it is determined that your research needs HSRC approval you must apply for AND RECEIVE notification of approval PRIOR to beginning your research.
