STUDENTS MUST MEET THE FOLLOWING CRITERIA:
- Attend one of our participating schools.
- Be at least 11 years of age.
- Meet low-income guidelines OR be a first generation college student (Both parents did not receive a 4-year college degree).
Application
- Download and print one of the following applications above.
- PARENTS: Complete pages 1 & 2- don't forget to sign the bottom of pages 1 & 2 with black or blue pen.
- STUDENTS: Complete pages 3 & 4 - don't forget to sign page 4 with black or blue pen.
- Attach a copy of pages 1 & 2 of parent's 1040 tax return.
- Attach a copy of the student's Social Security Card.
- Attach a copy of student's most recent high school transcript.
- Attach a copy of student's updated high school class schedule.
- Provide documentation if the student is under legal guardianship of someone other than biological parents.
- Mail the completed & signed application with a copy of pages 1 & 2 of parent's 1040 tax return and copy of the student's Social Security Card.
