COVID-19 Medical Vaccine and Booster Exemption Requests

Chico State Students:

If you are requesting a medical exemption to the COVID-19 vaccine or booster requirement, you must complete the following steps immediately.  

  1. Register with Accessibility Resource Center (ARC). Click the red "Apply Here!" button on the left to complete the registration application. Make sure to select "Vaccination Accommodation" under Question #1.
  2. Provide supporting documentation from your medical provider. You will be prompted to upload documentation upon completion of the ARC application, or you may email documentation directly to arc-vac-exemption@csuchico.edu.
    • Your medical provider may provide a note on their letterhead or use this Disability Verification Form (PDF). In either case, documentation from your medical provider must verify a diagnosis and substantiate the need for a medical exemption (in other words, your medical provider must indicate the impact to your health that would be caused by getting the vaccine).
  3. Self-certify for a medical exemption in the Student Center of your Chico State portal. 

If you have a current medical exemption to the COVID vaccine approved through ARC, and it has not expired, you do not need to request another exemption for the booster and you do not need to submit another medical exemption request in the Student Center.

Students who wish to request a religious exemption to the COVID vaccine or booster, or have religious exemption questions, must contact the Title IX/DHR office for next steps. ARC does not process or facilitate religious exemption requests. 

Chico State Faculty and Staff:

If you need to request a medical exemption to the COVID vaccine or booster, please email arc-vac-exemption@csuchico.edu as soon as possible. You will be contacted for next steps and provided with documentation to complete. 

Faculty and staff who wish to request a religious exemption to the COVID vaccine or booster, or have religious exemption questions, must contact Labor Relations. ARC does not process or facilitate religious exemption requests.
