Which forms does an employee submit if not transferring the fee waiver to a dependent?

The following matrix summarizes forms needed each semester, organized by fee waiver option and HR work flow.  Detailed instructions for employees are available by selecting a specific Fee Waiver Option.

Employee Admitted As Undergraduate or Graduate Admission?   Which forms should be turned into Human Resources each semester? If Approved...

Matriculated Student

Will be Career Development Option if pursuing a degree, certificate or credential; Career Development Plan (CDP) must be on file prior to submitting fee waiver application

No degree, taking undergrad courses OR has a degree and taking undergraduate courses OR has a degree and pursuing a 2nd undergrad degree = undergrad admit
First time only: separately apply for admissions
HR will collect and forward to Undergraduate Admissions
Has degree and taking grad courses = grad admit
First time only or after a break in enrollment: separately apply for admissions and submit a Request for Temporary Waiver of Graduate Application Fee Form
HR will collect and forward to Graduate Studies

Transitory Student

If Career Advancement Option, Career Development Plan (CDP) must be on file prior to submitting fee waiver application

If Job-Related Coursework,  Career Development Plan (CDP) not needed

No degree, taking undergrad courses = undergrad admit HR will collect and forward to Undergraduate Admissions
Has degree and taking undergrad courses OR has degree and taking grad courses = grad admit
