PHN Certification

You must first have a California registered nurse license before obtaining your PHN certification. You must be in possession of a valid California Public Health Nurse Certificate issued by the Board of Registered Nursing to be considered a Public Health Nurse. To become certified, thoroughly review and follow these steps:

1. Complete the BRN BREEZE online Application for PHN Certification

Updated information pending BRN communication.

Note: Use of the online process for applying for PHN certification and fee payment still requires completion of steps 2 and 3 as stated below.

2. Complete the BRN Request for Transcript-PHN Certification Form 

Mail the BRN REQUEST FOR TRANSCRIPT PUBLIC HEALTH NURSE CERTIFICATION (PDF) (found on pg. 7) form to the CSU, Chico School of Nursing using the following contact/address information. Once complete, the School of Nursing will forward your form to the BRN Advanced Practice Unit to be matched with your official transcript.

3. Request Official CSU, Chico Transcripts.

Order an official transcript from the Student Records and Registration Office Transcript Request page to be sent to the BRN PHN unit.

Note: New grads from either the basic BSN or RN-BSN program must wait until transcripts include all completed course work and reflect the nursing degree awarded and date conferred.
