PHN Certification

Follow these steps to be certified as a Public Health Nurse. You must be in possession of a valid California Public Health Nurse Certificate issued by the Board of Registered Nursing to be considered a Public Health Nurse. 

1. Request Official CSU, Chico Transcripts.

Order an official transcript from the Student Records and Registration Office Transcript Request page to be sent to yourself. Do not open the transcript when it arrives. 

2. Complete the BRN Certification form

  • Application for California Public Health Nurse (PHN) Certification (PDF)
  • For the Public Health Nurse Education section:
    • Name of Academic Program: CSU, Chico School of Nursing Chico, California, and USA.
    • In the right hand box: Check "Baccalaureate Degree".
    • Add the date you entered the nursing program, and the date of your graduation.
  • For the section regarding verification of child abuse/neglect prevention training, fill in the following:
    • CE Provider/School name:  CSU, Chico.
    • Course Name and Number: Include the following statement, “The content of CSU, Chico curriculum includes the child abuse/neglect prevention training.”
    • Number of Hours:  Not needed.  Leave blank.
  • Complete the Background Information section as instructed
  • Add your photo, sign and date the application.

3. Mail

Mail the APPLICATION FOR PUBLIC HEALTH NURSE (PHN) CERTIFICATION section to the BRN. Attach $300 fee with unopened official CSU, Chico transcript.

Mail the REQUEST FOR TRANSCRIPT PUBLIC HEALTH NURSE CERTIFICATION section to the School of Nursing. Once complete, the School of Nursing will send the form to the BRN.
