Open Enrollment is your once-a-year opportunity to evaluate and make necessary changes to your benefit programs. It is an opportune time to view health costs and plan changes and decide what best fits your needs.

Benefits Open Enrollment forms can only be accepted between Monday, September 20 through Friday, October 15, 2021 by noon.

Important Changes

PERS Care, PERS Choice and PERS Select plan names, premiums and coverages have changed for 2022. If you take no action, you will automatically be enrolled in the medical PPO plan equivalent to your 2021 enrollment. Click on 2022 Insurance Premiums HMO and PPO Plans or your current plan name below for more information.

Important Reminders

The Health Care Reimbursement Account (HCRA) and Dependent Care Reimbursement Account (DCRA) require re-enrollment every year during Open Enrollment to continue participation. Click on How to Enroll or Make Changes below.

If you’d like to start or stop participating with the MetLife Legal program, you will need to contact MetLife directly at 800-438-6388 during Open Enrollment.

  • What Am I Enrolled In?

    You can check your current enrollment for medical and dental plans by signing into your CSU portal(opens in new window). Next go to “Benefits Information” and click on “View Benefits Summary.” Then under “Plan Description” you can see what medical and/or dental plan you’re enrolled in.

  • PERS Care PPO (Now PERS Platinum)

    If you’re currently enrolled in PERS Care, here are some of the 2022 changes that could affect you.

    Effective January 1, 2022, the PERS Care PPO plan will be eliminated and all enrolled participants will be transitioned to the PERS Platinum PPO plan.

  • PERS Choice PPO (Now PERS Platinum)

    Effective January 1, 2022, the PERS Choice PPO plan will be eliminated and all enrolled participants will be transitioned to the PERS Platinum PPO plan.

    The Good News

    The new PERS Platinum PPO Plan will cover 90% of allowable charges after members meet their $500 calendar year deductible. As compared to the current PERS Choice PPO Plan which covers 80% of the allowable charges.

    Important:

    Unless you make a health plan change during Open Enrollment, you and any dependent(s) will automatically be enrolled in PERS Platinum PPO and you will see the premium increase on your December paycheck issued on 01/01/2022.

    Please Note:

    Also, if you’re considering the other two health plans offered by the CSU (Blue Shield Access+ HMO or PERS Gold PPO) these are both California only plans. If you (or your covered dependents) reside or obtain services outside of California, only emergency services are covered.

  • PERS Select PPO (Now PERS Gold)

    If you’re currently enrolled in PERS Select PPO, here are some of the changes that could affect you.

    Effective January 1, 2022, the PERS Select PPO plan will be eliminated and all enrolled participants will be transitioned to the PERS Gold PPO plan. The plan premiums will remain the same for 2022.

    PERS Gold Calendar Year Deductible:
    • Individual: $1,000
    • Family: $2,000

    **You will receive up to $500 in credits toward your Calendar Year Deductible if you complete the following activities:

    • Biometric Screening: $100 Deductible Credit
    • Flu Shot: $100 Deductible Credit
    • Non-Smoking Certification: $100 Deductible Credit
    • Second Opinion: $100 Deductible Credit
    • Condition Care Certification: $100 Deductible Credit

    If you qualify for all five credits, your 2022 deductible is reduced to: $500 for an individual (instead of $1,000). $1,000 for a family (instead of $2,000).

    (All preventive care screenings will count towards the biometric screening credit. Any flu shot within the past 18 months will be credited in the calendar year. Non-Smoking credit will be given unless you are identified as a smoker. Second opinion will be credited unless you have surgery without a second opinion. Condition Care will be credited, unless you are contacted by a nurse and decline to participate in Anthem’s clinical management program).

  • How to Enroll or Make Changes

    Medical and/or Dental

    If you’d like to enroll or make changes to your medical and/or dental plans, please complete a Benefits Enrollment/Change Form (PDF).

    You can submit this form during Open Enrollment, either via email to benefits@csuchico.edu. Or you can drop it off at the Benefits Office located in Kendall Hall, Room 213, Monday – Friday from 8:00 am to 5:00 pm.

    If submitting electronically, please do not submit any documents with Social Security numbers on them, as e-mail is not secure.

    If you’re making a plan change only, there is no need to submit marriage/birth certificates with the Benefits Enrollment/Change Form, as we already have these documents on file. Please be sure to list everyone on your plan in Section F to make sure no one is excluded from the plan.

    If you’re enrolling for the first time or adding a new dependent, please see the required documentation listed on the Benefits Enrollment/Change Form.

    Premier Vision Plan (VSP)

    Benefit eligible employees are automatically enrolled in the CSU paid VSP Basic Family Plan.

    For an additional cost you have the option to enroll in the VSP Premier Plan(opens in new window). This enrollment process needs to be done directly with VSP during Open Enrollment. (Note, if you have dependents on your plan, to keep their vision benefits, you’ll need to enroll all of your family members in the Premier Plan otherwise they’ll lose their vision benefits).     Health Care Reimbursement Account (HCRA) and Dependent Care Reimbursement Account (DCRA)

    To start or continue participation with HCRA/DCRA, please complete the 2022 Health and Dependent Care Reimbursement Account Form (PDF). You can either email the completed form during Open Enrollment to benefits@csuchico.edu or you can drop it off at the Benefits Office in Kendall Hall, Room 213.
