Benefits Open Enrollment forms can only be accepted between Monday, September 21 through Friday, October 16, 2020 by noon.

Open enrollment is your once-a-year opportunity to evaluate and make necessary changes to your benefit programs. It is an opportune time to view health costs and plan changes and decide what best fits your needs. If you do nothing, your medical, dental, and vision enrollment will continue into the 2021 plan year.

Required Documents
Due to COVID-19 restrictions, we are requesting that all Benefits Enrollment/Change Forms (PDF) to be submitted to benefits@csuchico.edu via a scanned document. Please do not submit any documents with Social Security numbers on them, as e-mail is not secure.
If you are making a plan change only, there is no need to submit marriage/birth certificates with the Benefits Change Form, as we already have these documents on file. Please be sure to list everyone on your plan in Section F to make sure no one is excluded from the plan.
Ways to submit documentation
- Benefits forms, signed through Adobe Sign, emailed to benefits@csuchico.edu. (Not sure how? Employees can download software, and log in…) Submit ticket(opens in new window) if you have questions on Adobe Sign.
- Scanned Benefits forms, completed, with signature, emailed to benefits@csuchico.edu.
- In person during limited walk-in days and times. Benefits staff will be at the Information Center:
- Tuesday: 2pm - 5pm
- Thursday 9am - 12pm

Join us this year for a Virtual Open Enrollment & Benefits Fair on Thursday, October 1, 2020, between 1pm-4pm. During this Benefits Fair you will be able to chat with all participating vendors, representatives from CalPERS and the Chancellor’s Office HR team, as well as Chico State’s Benefits team. Once registered, you will have access 24/7 to the information on the Benefits Fair website, but will only be able to chat with vendors during the actual event. The Dependent Care Reimbursement Accounts and Health Care Reimbursement Accounts require annual re-enrollment. Please complete the 2021 Health and Dependent Care Reimbursement Account Form (PDF) and submit it to the HRSC Benefits Office, [Virtual process]. Benefits Open Enrollment forms can only be accepted between Monday, September 21 through Friday, October 16, 2020 by noon.

Retirees
The Chancellor’s Office is the benefits administrator for all retiree benefits. During Open Enrollment and throughout the year CSU retirees can access health, dental, and vision information online by going to CSU Retirees(opens in new window) website. Questions should be directed to CSU Systemwide Benefits office at 562-951-4411.

- The Plans and Rates(opens in new window) is a one-stop-shop for current subscribers of the Basic PERSCare, PERS Choice, and PERS Select health plans.
