COVID-19 Self Reporting Form 

Staff who test positive for COVID-19 should now report their case via Chico State's online reporting form(opens in new window). The self-reporting platform replaces the COVID-19 hotline, which will close on February 28. Only COVID-19-positive tests need to be reported (not symptoms or exposures).
