Max Unit and Satisfactory Academic Performance (SAP) appeals can be printed from PDFs on this webpage. Most financial aid forms are completed electronically through your Financial Aid Account(opens in new window). If for some reason you are unable to create a Financial Aid Account or upload documents, please contact our office for assistance at 530-898-6451.
A few forms for the 2023/2024 aid year are provided in Adobe Acrobat PDF format. Download the free Acrobat Reader if you don't already have it.
Appeals
- Satisfactory Academic Progress Appeal (PDF)
- Maximum Unit Appeal (PDF)
- Childcare/Elementary/Secondary School Tuition, Travel Adjustment, or Student Change in Marital Status Appeals
- To request one of these appeals select the "Manage Requests" button at the top right corner of your Financial Aid Account and click the plus sign next to the appropriate appeal. If you have not logged into your Financial Aid Account before, you will need to create an account. (To create an account, enter your name exactly as it appears on your FAFSA or CADAA).
- After selecting the appeal enter a short one-sentence reason, then click submit.
- Under "Needs Action" click the box with your appeal.
- Click on the red bar.
- Click "Fill Out".
- Follow the instructions to complete the electronic form.
- Complete the electronic form and e-sign it by entering your password and clicking "E-sign". Click "Ok". If you or your parents are unable to e-sign the form, download it and sign it.
- Upload supporting documentation if required.
- Finish your appeal by clicking the “submit” button at the bottom.
- Appeals are reviewed in the order received. You will receive notice of our decision in your Wildcat email. For more information visit our appeals web page.

- Income Reduction or Non-Reimbursed Medical/Dental/Vision or Support of Extended Family Members Appeals
- To request a this type of appeal select the "Manage Requests" button at the top right corner of your Financial Aid Account and click the plus sign to the right of " Professional Judgement: Special Circumstances - EFC Calculation Appeal". If you have not logged into your Financial Aid Account before, you will need to create an account. (To create an account, enter your name exactly as it appears on your FAFSA or CADAA).
- Enter a short one-sentence reason for your appeal, then click submit.
- You will see the message "Your request has been successfully created!" Click "Ok".
- Under "Needs Action" click the "2023-2024 Professional Judgement" box.
- Click the red bar with the words "Exceptional Circumstances Appeal".
- Select the appropriate option
- Income Reduction, or
- Dependent Non-Reimbursed Medical/Dental/Vision Expenses, or
- Support of Extended Family Members
- Click "Fill Out"
- Follow the instructions to complete the electronic form and e-sign it. If you or your parents are unable to e-sign the form, download it and sign it.
- After submitting you appeal form be sure to upload any required supporting documentation.
- Finish your appeal by clicking the “submit” button at the bottom.
- Appeals will be reviewed in the order received. You will receive notice of our decision in your Wildcat email. For more information visit our appeals web page.

- Unusual Circumstances Appeal (Dependency Status Appeal) for 2023/24:
- To request an unusual circumstances appeal select the "Manage Requests" button at the top right corner of your Financial Aid Account and click on the plus sign to the right of " Professional Judgement: Unusual Circumstances Appeal". If you have not logged into your Financial Aid Account before, you will need to create an account. (To create an account, enter your name exactly as it appears on your FAFSA or CADAA).
- Enter a short one-sentence reason for your appeal, then click submit.
- You will see the message "Your request has been successfully created!" Click "Ok".
- Under "Needs Action" click the "2023-2024 Professional Judgement" box.
- Click the red bar with the words "PJ Unusual Circumstances Appeal".
- Click "Fill Out".
- Follow the instructions to complete the electronic form.
- Click "I will provide supporting documentation" and click "Continue".
- Enter your password and click "E-sign" and then click "Confirm" on the Terms and Conditions page.
- Upload supporting documentation.
- Finish your appeal by clicking the “submit” button at the bottom.
